During the first visit, we make sure to obtain important background information, like your medical history, and give you time to get to know your doctor. We accept most insurances.

The Family Foot Health Center, doctors and staff, will work with the patients together to realize a shared vision of uncompromising excellence in podiatric care.

New Patients:

1. Please use the patient portal link emailed to you when you made your appointment.
2. New patients must arrive 30 minutes prior to appointment time or be rescheduled. This allows our staff the time needed to complete your check in process and update your patient chart.

Please bring the following items to your visit:

• Patient's insurance card (virtual cards not accepted)
• List of current medications and/or over the counter medications, including dose and frequency
• Information about patient's medical and surgical history
• Recent test results, x-rays, or relevant records

Insurance and Payment Information:

On the day of your appointment, you are responsible for co-payments or charges which are known to not be covered by your insurance. If your deductible has not been met, your portion of all charges will be collected.
